Consent

Speak Up, a program of Fraser Health, collects and protects your personal information under the authority of the Freedom of Information and Protection of Privacy Act (FIPPA).  The purpose for collecting personal information through Speak Up is to provide youth and young adults (13 to 25 years of age) with an opportunity to share their ideas and experiences with others, enabling youth to have a voice in mental health. 

The type of personal information collection that you are being asked to consent to is the following, depending on where in Speak Up you choose to provide this information:

  • Your name or alias
  • Email address
  • Internet Protocol (IP) address
  • Your thoughts, stories, original art, photos, videos or music.

With the exception of your name, email address and IP address, your information will be shared with others who access the Speak Up webpage via the Internet.

Your name, email address and IP address will only be accessed by Mental Health and Substance Use Program staff at Fraser Health who look after Speak Up, and who have received privacy training. 

If you choose to access Social Media – Twitter and Facebook - through Speak Up you are advised to review their privacy policies prior to submitting any personal information.

Your personal information collected in Speak Up will be securely stored in BC and, when no longer needed, safely destroyed as per Fraser Health information policies.

You will be asked to complete this consent each time you wish to contribute personal information to Speak Up.
